We have an automated telephone booking service which you can access 24 hours a day by ringing the telephone number above. You will be asked to confirm your date of birth and the main telephone contact number we hold for you.
If your condition is non-urgent, you can expect to see a GP within two working days, though you may have to wait longer if you want to see a particular GP.
Courtside Surgery is pleased to have been accredited by the Severn Deanery as a GP Training practice.
This means the practice will have attached to it a qualified doctor training to specialise in General practice. Each doctor will remain with us for a period of 6 or 12 months. You may be offered an appointment with the Registrar or asked if you would mind them sitting in with your GP.
You may also be asked if you would mind the consultation being video recorded. This is purely for the educational training purposes of the doctor concerned. The videos are used strictly according to Royal College of General Practitioner’s guidelines and are destroyed within 12 months.
For your convenience it is normally possible to book an appointment up to four weeks in advance. Let us know if more than one person in the family needs to be seen. We can give you a longer appointment if necessary.
Tell us if you want someone to accompany you during an examination, or a private room to discuss any matters. Remember that the results of tests can only be given to the patient.
You can book to have a late morning telephone consultation with a doctor. He or she will ring you on the telephone number you have given.
You can help us by
- Being on time for your appointment.
- Letting us know if you need to cancel.
- Calling for a home visit or urgent appointment before 10.00 am
- Ringing for the results of tests after 2.00 pm
If possible please try to telephone reception before 10am if you require a home visit.
A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
House visits are only available for patients who are housebound because of illness or disability.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.
Please call after 2:00pm to enquire about your test results as our reception staff will have more time to deal with your request between these times.
Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results.
When you take your test you will be told how long it will be before the results are returned to the practice.
It is your responsibility to check your results and to make an appointment to discuss them with your doctor if your are advised to do so.
Please use the tear-off slip to request repeat prescriptions and bring, fax or post it to us. You may also request repeat prescriptions online via our website.

Your prescription will be ready for collection at the local pharmacy of your choice after 3.00 pm two working days after we receive it. Prescriptions cannot normally be collected from the surgery.
We apologise for the sudden loss of our previous online repeat request form. The current system has been in place for some months and provides increased security and makes it easier to select the medications you require. It also provides online appointment booking.
To use the service, you do need a username and password. To register, click on the link above, then select ‘I want to sign up’. Complete and submit the online form, then for your security, we will post your login details to the address we hold for you. You should receive it within 3 working days.
New Patient Registration
If you live in the Yate area and would like to register with Courtside Surgery, please complete one of our registration forms that are available from our reception.
On the form, you will be able to say which practitioner you would prefer to see. However, you will be registering with the practice rather than an individual GP.
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the “Send” button to mail your form electronically to us. You will still need to visit the surgery to sign a paper copy of the form to confirm that the details are true and correct. you will also need to provide photographic proof of your identity, date of birth and address before we can fully register you and before you can make any appointments.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct. you will also need to complete a Patient Profiling form (see below) and bring that with you.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out, sign it and bring it to the surgery with proof of your identity, date of birth and address (see above table). Please also complete the Patient Profiling form.
