Sickness certificates (fit notes)

You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home visits

In cases where you are too ill to attend surgery, a home visit can be arranged. If a visit is required, please telephone the surgery on 01454 313874 before 10:30am wherever possible.

This enables the doctors to plan their workload efficiently and avoids the possibility of two doctors having to drive separately to the same part of town or the same village.

Please help us to help you. A home visit is very time-consuming, so we respectfully ask that you only request a visit if no other alternative is available. Remember that you are likely to be seen more quickly if you can attend the emergency surgery than if you wait for a home visit.

On average, we can see four or five patients at the surgery in the time taken to do one home visit. After midday, there is only one doctor on duty for visit requests. These “late requests” should be restricted to unforeseen emergencies or problems that could not reasonably wait until the following day.

Change or cancel an appointment

If you can’t attend your appointment, contact us as soon as possible to cancel. This is so we can offer your appointment to someone else in need.

To cancel your appointment:

  1. Use your NHS account (through the NHS website or NHS App)
  2. Use the text message function, when you receive a text reminder
  3. Phone us on 01454 313874 – Monday to Friday from 8am to 6:30pm
  4. Walk in during front desk opening hours 8am to 11:30am and 2:30pm to 6pm

It this is something you would prefer, please request this upon booking and we will try our best to accommodate.

Pre-booked appointments, either face-to-face or telephone, are available from 7:30am to 8pm daily.
